Register for ISBT 128 with ICCBBA

Three steps to make the ISBT 128 connection

Thank you for your interest in becoming a member of the global ISBT 128 network.

ICCBBA, Inc licensees are permitted to make use of the ISBT 128 standard in data collection, labeling and processing of blood, tissue and organ products to achieve the highest standards of accuracy, safety and efficiency.

There are three steps to registering to become an ICCBBA Licensee:

(1.) Download the ISBT 128 Registration Form 

Download the Registration form by clicking here. Vendors should use a separate Vendor Registration Form. Right click to save the PDF file to your desktop or load the PDF document in your browser and then print it out. To view the PDF document in your browser or on your desktop, you may need to install the free Adobe Acrobat reader.

(2.) Complete the ISBT 128 Registration Form

All applicants should complete part A of the form. Facilities requiring allocation of one or more Facility Identification Numbers (FIN) should complete one copy of Part B for each FIN required.

(3.) Mail or Fax the ISBT 128 Registration Form

Submit the form together with the required payment (if paying by check) by mail to: PO Box 65253, Charlotte, NC 28265-0253, USA.

If paying by credit card or wire transfer, you may fax the form to the ICCBBA office at (+1 909 793 6214).

Note that on completion of the registration process, the first year Annual License Fee becomes due and will be invoiced.

Please note:  It is currently taking three to five weeks to process registration forms due to the high volume of forms ICCBBA is currently receiving.  Your facility is important to us and will be processed in the order it is received.  You will receive notification from ICCBBA via a welcome letter and CD that is mailed to the facility once the registration form has been processed.  This letter is sent through the United States Postal Service.  If it has been longer than the three to five weeks mentioned, please contact our office so we can research the delay.  Thank you for your patience and good luck with the implementation of ISBT 128!
